It is a Type 18/24 spring brake chamber, combining a 18-square-inch service diaphragm for regular braking and a 24-square-inch emergency/parking section with a powerful spring. When you step on the brake, air fills the service side and pushes a rod to apply the disc caliper; the spring side automatically locks the brakes when air pressure drops. You replace it when the chamber rusts through, leaks air, or the spring defaults.
