This is a left-hand threaded inner cap nut used in stud-piloted (Budd‑type) dual wheel setups on heavy‑duty trucks.

It threads onto the wheel stud first, securing the inner wheel, and provides an external 1‑1/8″‑16 thread for the outer cap nut. Left‑hand threads are used on the driver’s side to resist loosening during forward motion. The nut is typically replaced when threads are stripped, corroded, or during routine wheel‑end service.
