This is a left-hand thread, double-ended wheel stud used to secure dual wheels and brake drums on heavy-duty truck and trailer axles.

It has a clip collar (shoulder) that holds the stud in place in the hub during assembly. Left-hand thread studs are traditionally installed on the left (driver's) side of the vehicle to help prevent lug nuts from loosening due to wheel rotation. You would replace a stud like this when threads are damaged, stripped, or if a stud has broken during service.
